Patient Evaluation Board (Procedure)
A board in Cancer Patients Welfare Society GINUM has been working since 2010 in which evaluation of patients is performing after approval of the board, poor & needy patients are being registered according to the decide share for chemotherapy medications, facilities for diagnosis & post medications. Following procedure is adapted for the evaluation of needy / poor patients :
Step 1 (Pre-Evaluation) :
When patient comes in OPD the duty oncologist pre evaluates the patient in which name of the chemotherapy and total number of cycles has been prescribed & refer poor patient to CPWS (Pharmacist CPWS).
Step 2 (Evaluation) :
As the patient referred by the duty Oncologist come to the Pharmacist CPWS here its complete evaluation included patient biodata, source of income, dependent\in dependent, financial status is discussed in a proper designed Performa and a case file is formed. In this file complete detail of cost per cycle of prescribed Chemotherapy and total cost for the whole treatment is mentioned in detail. In this discussion patient is encouraged to share almost 50 percent of the total estimated cost.Patient is asked to come on meeting day for final approval & given reminder of the meeting in written form.
Step 3 :
In case of emergency Director GINUM (Sr. Vice President of CPWS) is fully authorized to issue 1st cycle of chemotherapy to the patient without any delay in the treatment of the patient without any wait in conduction of board meeting.
Step 4 :
All the files of the referred patients are made through the said procedure. Office Secretary of CPWS is responsible to collect all the files from Pharmacist & arrange them in CPWS office on daily basis.
Step 5 :
Office secretary is responsible to do all the arrangements in order to conduct the board meeting on every Tuesday with coordination of pharmacy Assistant. i.e
• Call to all Patients regarding meeting & insure their presence.
• Call & Message to Patient Evaluation Board Members.
• Making agenda of the meeting.
• Make a Circular to inform the concerned.
• Arranging the GINUM & CPWS files according to medical condition of patients.
• All other necessary actions for the meeting.
Step 6 :
On meeting day all the board members attend the meeting and approval is given to all referred patients and conducted interview of patients one by one by mutual agreement. Patient evaluation board members and patient/attendant sign the file.
Step 7 :
After approval of Patient Evaluation Board patients can take treatment according to approval.
